Please download and print this form, then turn it in to the church office or in the black tray in any of Ascension’s youth rooms. We need to have this on file for all of our youth; this is what makes you “one of ours” so we can take you along on special events! Anyone who is going to join us for anything other than a Sunday morning class meeting needs to get one of these filled out and signed by a parent.
Basic Registration Form, Group Dynamix Form, and Our Savior Form for the 30 Hour Famine
All participants in our April 10-11 30 Hour Famine overnight need to have each of these three sheets filled out and returned to Stephen, along with the $30 fee (or a scholarship).
2009 Navajoland Mission Trip Registration Form

Interest Form for Drama, Readings, Music
The leaders of the Worship Ministries at Ascension want to offer our youth opportunities to put their gifts to God’s service in worship. Lots of our young people perform in choirs, orchestras, bands, drama and dance groups. Others have other artistic talents or gifts for reading or leading prayers in public. You can use this form to let the Worship Ministries team know your interests and gifts so they can help you use them in God’s service here at Ascension.
When other forms and resources are needed (such as registrations for special events like retreats and mission trips), this is the page where they will be posted.
